Speech Language Pathologist (SLP)

Role Overview:

The Speech Language Pathologist (SLP) provides direct speech therapy rehabilitation to patients in skilled nursing facilities, focusing on improving functional outcomes and quality of life within a patient-centered environment.

Key Responsibilities

• Assess, plan, implement, and modify speech therapy treatment plans based on physician referrals and patient needs.
• Set measurable, realistic goals for patient progress and evaluate outcomes.
• Chart therapy sessions accurately and timely, utilizing therapy software.
• Collaborate with interdisciplinary teams, including nurses, therapists, and medical staff.
• Supervise and evaluate speech therapy assistants or Clinical Fellowship Year (CFY) speech-language pathologists, if applicable.
• Participate in community health initiatives and maintain professional relationships.
• Adhere to all laws, regulations, and facility policies, ensuring patient safety and confidentiality.

Qualifications \& Skills

• Graduate of an accredited speech therapy program (MS or MA).
• Current licensure in applicable state and ASHA certification (CCC) or eligibility for CFY.
• Preferably one year of experience; new graduates are welcome to apply.
• CPR certification required or obtained during orientation.
• Proficiency in Microsoft Office and excellent communication skills.
• Ability to work independently, demonstrate good body mechanics, and maintain professionalism.

• Establish, assess, and modify treatment goals.
• Document patient progress and utilize therapy software appropriately.
• Provide excellent patient service and collaborate effectively with team members.
• Maintain attendance, demonstrate good judgment, and adhere to confidentiality and compliance standards.
• Perform physical activities such as sitting, standing, lifting, and reaching during work.
