Position Summary: The team lead will provide direct clinical services to the clients to assist with assessment and treatment planning (IM CANS), independent living skills, case management, crisis management, and medication support. The Team Lead coordinates and monitors the delivery of appropriate medical and case management services in a residential setting serving approximately 16 adults with mental illness.
Hours of Employment: The manager will determine the hours of employment based on need. The normal work week is forty hours, Monday through Friday 9:00am-5:00pm. The position requires eight to ten hours per day except when adjustments are needed to accommodate the specialized needs of the department including medication passes.
Days of Employment: The manager will determine days of employment based on departmental need. Flexibility to work varied hours may be required.
Essential Duties
- Check in medications daily
- Med roomorganization
- Daly review of EMARs
- Pill counts
- Primary liaison with medical and psychiatric doctors for appointments
- Create and manage house calendar of events/activities/groups
- Create and manage client calendar of appointments
- Coordinate transportation needs of clients
- Create a daily house chore list for clients. Follow up for implementation.
- Check in medications daily
- Run report and evaluate medication errors, recommend improvements to systemsto decrease errors
- Oversee portal with providers like Access
- Work with Tarrytown pharmacy to ensure residents have medications per their doctor’s orders
- Train staff in theappropriate medicationsupports with membersper Anixterguidelines.
- Helptomonitoreating habitsasappropriateorwhen on modified diets for medical reasons.
- Conduct MH assessments and treatment planning at a minimum every 6 months (IM CANs) and enter into the electronic health record system.This duty is shared with the manager.
- Assistclients withmedical appointments,or arrangeappointments to be coveredbyother staff,providing transportation as needed.
- Observe and reportany changes in the client’s physical, ormental illnessto serviceteam, not limited to psychiatrist, medicaldoctor, and guardian.
- Ensurethatrequireddocumentation is completeand/or uploaded to the member’s EHRandin compliance withstate/ federalregulations andCARFstandardsusingAWARDS /Foothold (ElectronicHealthRecordsystem).
- Attendall staff meetings,trainings, and educational classes asrequired.
- Coordinatemedical appointments to ensure compliance withCARF,Rule 132/ Rule 140including but not limited toaccompanyingtheresidentsto appointments.
- Activelyparticipatein the interdisciplinary team processassistingin the formulation oftreatmentobjectives:
- Coordinate/conductassessments of each client’s medical needstoestablishappropriate priorities.
- Coordinate/conductinstruction to program manager/staff on methods and techniques in delivering medical care and treatment.
- Ensure that each client’s progress in response to medical carerenderedis documented as needed.
- Collaboratewith other members of the interdisciplinary teamwhen medical condition affects daily living / programming.
- Establish,implement,andrevise, as necessary, medical care treatment policies and procedures.
- Functionasthe primary liaison between each medical practitioner conveying relevant data to ensuretimelyandappropriate changesin each client’s medical care plan; communicating changes to the interdisciplinary team, and all other pertinent parties.
- Participate in the emergency on callrotation;6–7timesper year(for two weeks at a time).
- Assistwith schedules and coverage of 24/7home, act as point person when manager is unavailable.
- Help participants with personal care functions as needed, including sharing doctor’s instructionsregardingneeds and follow up with manager and staff (i.e.information on bandages, medications, ointment, wound care, ambulation, diet, etc.)
- Provide direct care for non-medical issues such as counseling, activities of daily living, budgeting, meal prep,hygiene.
- Document services weekly (dailyis preferred) meeting target ofa minimum of22 hours / week.
- Step in for manager when they are out of the setting or unavailable.
- Other duties as assigned.
Essential Skills and Experience:
- Meet definition for Mental Health Professional per Rule 132
- IM CANs certified within 2 months of hire
- Minimum of twoyears clinicalexpertisewithspecialized coursework/experience working with personswithdisabilities(preferably psychiatric) andexperience in medical/social service field.
- Must havevaliddriver’s licenseand drive agency vehicle.
- Excellent interpersonal and communication skills.
- High performance teams anda strongteam player.

- Computer proficient.
- Experience in medical care and documentation of individuals with disabilities, particularly serious mental illness, per Rule 132\.
