Job Description

We’re looking for a motivated and enthusiastic Occupational Therapist to join our Intermediate Care service, working across community and hospital pathways to support people to regain independence and return home safely.

This is a great opportunity for a newly qualified or experienced Band 5 Occupational Therapist to develop their skills within a supportive, forward-thinking team. You’ll

As a Band 5 Occupational Therapist within the Intermediate Care service, you will manage a varied clinical caseload, supporting people to regain independence and achieve safe and timely discharge from hospital or community pathways.

In This Role, You Will

  • Manage your own caseload
  • Carry out holistic assessments and identify needs and risks
  • Plan and deliver person-centred interventions
  • Work closely with the wider team to support discharge and flow
  • Prioritise work and respond to changing demand
  • Keep clear, accurate clinical records
  • Delegate appropriately to therapy assistants and support their development

  • Can manage their own workload and prioritise effectively in a fast-paced environment
  • Demonstrates sound clinical reasoning and a willingness to learn and develop

  • Is flexible and able to adapt to the needs of the service
