Activa Home Health is actively seeking experienced Registered Nurses for our innovative

Hospital at Home Program

We are seeking highly skilled RNs to deliver

acute-level care in the home setting

, supporting patients who would otherwise require hospitalization. This role is ideal for nurses who thrive in dynamic environments, deliver high-acuity care in the home while completing timely, accurate clinical documentation in Kinnser (WellSky), and are passionate about advancing patient-centered care models.

Key Responsibilities

• Provide high-quality, acute-level nursing care in a home setting as part of the Hospital at Home program
• Collaborate with interdisciplinary
• Monitor patient status and respond to changes in condition with strong clinical judgment
• Deliver advanced clinical interventions while ensuring patient safety and satisfaction
• Accurately document care in electronic medical records (EMR)
• Adhere to program workflows, response expectations, and professional standards

Required Qualifications

• Active Florida Registered Nurse (RN) license
• Recent hospital-based experience required (Acute Care, ICU, ER, or Step-Down)
• Experience in high-acuity home health or complex medical care strongly preferred
• Proven ability to collaborate within interdisciplinary clinical teams
• Strong critical thinking, assessment, and communication skills
• Comfort with mobile technology and electronic documentation systems
