Nurse Practitioner [Spanish Required]

We are seeking a

Nurse Practitioner

to deliver effective, equitable primary care to adult Medicare patients in an outpatient setting.

Responsibilities include:

providing routine and acute office visits, conducting wellness assessments, care coordination with other providers, leading population health initiatives, patient education, phone triage, and home visits when needed.

Qualifications:

Master’s or Doctorate in Nursing, national certification in Family, Adult-Gerontology, or Gerontological Nurse Practitioner specialties, active NP and DEA licenses, US work authorization, and bilingual proficiency in Spanish.

Preferred skills:

experience in primary care, knowledge of HCC documentation, ICD-10 coding, and passion for team-based, population health-focused care.
