Triumph Private Home Care is seeking a detail-oriented Registered Nurse to join our team in a part-time Clinical Documentation Review role. This position is responsible for reviewing clinical documentation to ensure accuracy, completeness, and compliance with Medicaid, agency, and state documentation requirements using our electronic medical record, Alora Plus.

This role is ideal for an experienced RN who enjoys chart auditing, attention to detail, training, and helping maintain high-quality clinical records.

Hours: Approximately 8–10 hours per week (Flexible Schedule)

Responsibilities:

• Review skilled nursing visit notes in Alora Plus for completeness, accuracy, and compliance
• Verify documentation includes appropriate narratives, interventions, medications, signatures, dates, visit times, and physician orders
• Review caregiver/personal care documentation and timesheets within Alora Plus
• Return documentation requiring corrections and communicate needed revisions to field staff
• Review and maintain nursing care plans, aide plans of care, and other clinical documents as needed
• Monitor expiring care plans and assist with timely updates in Alora Plus
• Ensure documentation is completed and approved prior to agency billing deadlines
• Conduct charting and documentation orientation for newly hired nurses and caregivers using Alora Plus
• Provide guidance and education to clinical staff regarding documentation requirements and best practices
• Monitor documentation trends and identify opportunities for quality improvement
• Work closely with office staff to maintain documentation compliance and audit readiness

Qualifications:

• Current Georgia Registered Nurse (RN) License
• Minimum of 2 years of nursing experience (home health experience preferred)
• Experience using electronic medical records (EMRs); Alora Plus experience is strongly preferred
• Experience reviewing clinical documentation or performing quality assurance preferred
• Strong understanding of home health documentation standards and Medicaid compliance
• Excellent attention to detail and organizational skills
• Strong written and verbal communication skills
• Current BLS Certification
• Clean criminal background check

Pay: $48\.00 - $50\.00 per hour
