Job Description
Founding Product Engineer — own the product at a YC-backed healthcare AI rocketship
Are you the kind of engineer who'd rather sit with a customer and figure out what to build than wait for a ticket? Have you shipped something real that people actually use — and felt the pull to own the whole thing, from the problem to the launch? Do you want to be one of the first engineers at a company that's already winning?
A YC-backed AI startup is hiring founding product engineers to build AI-native products for a massive, underserved corner of healthcare. They've got genuine product-market fit: 5x revenue growth in six months, $1M\+ ARR, 200\+ customer locations daily-active, 99% retention — and they've never lost a customer to a competitor. They're targeting 700-1,000 locations by year-end. This is a real rocketship, not a deck.
They're backed by CRV and Y Combinator, the founding team is tiny (you'd be one of the first engineers), and the work is a true PM \+ engineering hybrid — you'll meet the end users, define what to build, ship it, and own the outcome.
What you'll do
▸ Own the full product lifecycle, meet customers, identify their real pain points, design the solution, build it, launch it, and measure the impact. No layers between you and the user.
▸ Build and ship full-stack AI-native products (React, TypeScript, Python, Postgres) that genuinely change how your customers work day to day.
▸ Build and improve AI inference pipelines, agent loops, and evaluation systems powering their core products.
▸ Improve observability, reliability, and infrastructure to support rapid scaling for 200\+ locations heading toward 1,000\.
▸ Work directly with the founders to shape product strategy using analytics and direct customer feedback.
The founders
Two repeat builders from a top engineering-and-business program. The CTO took a security product from zero to $2M ARR at a company you've definitely heard of (now a low-nine-figure line of business) and led the LLM transition at a well-funded healthcare AI startup. The CEO built a widely-used AI voice product. Both are deeply embedded in the startup ecosystem and know how to ship. You'd be working shoulder-to-shoulder with them.
This is a 5-days-a-week in-person role in downtown San Francisco. The founders work hard (they're honest that this is a \~60-hour-a-week founding-team environment) and they're looking for people who want to be in the room building something that's already working.
What they're looking for
✓ 2-4 years of full-stack engineering (open to exceptional entry-level), with strong React/TypeScript/Python/Postgres and real-time or event-driven backend experience
✓ You've shipped a product end-to-end that real people have used
✓ Startup DNA and an entrepreneurial narrative
✓ You've built with AI/LLM pipelines
✓ Self-directed, high-ownership, intellectually honest, and genuinely product-minded where you think about user impact, not just code
✓ Genuinely interested in healthcare and building for non-technical users
What they offer
💰 $150K–$250K base \+ 0\.5–1\.5% equity — meaningful founding-level ownership
- A founding-engineer seat at a company with proven PMF and a clear path to 5-10x growth this year
- Repeat founders with real exits and scale experience to learn from directly
- True end-to-end ownership — you're helping build the team, not joining one
- A $200B market with a wedge product that's already winning, expanding into continuous patient care
- Visa: STEM OPT candidates welcome; note they're not sponsoring new or transfer H-1B visas at this time
