CTO & Co-Founder - Medical AI Imaging & Telehealth (No Salary, Equity Only)

ABOUT THE OPPORTUNITY

We're building a unified cloud-native platform for secure, scalable acquisition and AI-driven analysis of diagnostic medical images — engineered to democratize expert-level imaging interpretation across geographies, institutions, and resource-limited settings.

Our technology combines deep learning, federated learning, and real-time telehealth orchestration to give clinicians in rural hospitals, distributed oncology teams, and emergency departments the diagnostic capabilities typically reserved for top-tier academic medical centers. The platform supports DICOM/HL7/FHIR-native ingestion, multimodal collaboration, and regulatory-compliant data flows across jurisdictions.

We're entering a $22B global market at a moment when AI imaging, cross-border telehealth, and federated ML have matured enough to make our approach viable — and when geographic fragmentation in diagnostic expertise has never been more acute.

This is a co-founding opportunity. We're looking for a technical leader who wants to own what they build — someone who is driven to create, not just execute. As a founding member, you'll hold meaningful equity and shape every dimension of the product, technology, and company from day one.

THE ROLE

As CTO \& Co-Founder, you will translate our clinical mission into a federated AI and telehealth-enabled cloud platform — defining the architecture, AI engines, and compliance frameworks that bring expert imaging diagnostics to clinicians everywhere.

Your core responsibilities:

• Architect and build a cloud-native microservices platform for distributed medical image ingestion, processing, and interpretation.
• Design and deploy DICOM, HL7, and FHIR integration pipelines for seamless interoperability with hospital systems and PACS environments.
• Lead development of federated deep-learning pipelines and AI inference engines that preserve patient data locality.
• Define and build the multimodal telehealth UI and real-time clinician collaboration layer.
• Establish DevOps and CI/CD practices with HIPAA, GDPR, and regional medical data compliance built in from day one.
• Own security, secure provenance, audit trails, and data governance architecture.
• Drive patent and IP strategy for core AI and platform innovations.
• Recruit, lead, and mentor founding R\&D, ML, and DevOps teams toward pilot deployments and early design partner engagements.
• Partner with the CEO on investor technical diligence, product roadmap, and go-to-market infrastructure.

REQUIREMENTS

• 10\+ years of software engineering leadership, with at least 5 years in a CTO, VP Engineering, or Principal Architect role.
• Strong background in health-tech SaaS and medical imaging informatics (DICOM, PACS, RIS, HL7, FHIR).
• Proven experience designing and deploying federated machine learning systems and deep learning pipelines in production.
• Deep cloud architecture expertise across AWS, GCP, and/or Azure — including Kubernetes, Terraform, and infrastructure-as-code practices.
• Hands-on experience delivering HIPAA-compliant and GDPR-compliant systems, with working knowledge of SaMD (Software as a Medical Device) regulatory pathways.
• Strong grasp of cybersecurity, zero-trust architecture, encrypted data pipelines, and secure provenance.
• Track record of building and leading high-performance engineering teams from the ground up.
• Startup "zero-to-one" mindset — you thrive with ambiguity, scarce resources, and high leverage.
• Fluent English and excellent communication with clinical, technical, and investor audiences.

NICE TO HAVE

• Prior founding experience with a successful exit or significant fundraising track record.
• Published research, patents, or open-source contributions in medical imaging AI, federated learning, or computer vision.
• Experience with 510(k) submissions, CE Mark, or equivalent medical device certifications.
• Background in teleradiology, clinical workflow design, or PACS/EMR integration.
• Network of relationships in hospital IT, payers, or medical imaging device manufacturers.
