We are backed by tier-1 healthcare VCs, Techstars, American Cancer Society, and Moderna with early revenue and active partnerships across pharma and major health systems. Our founder, a Harvard graduate who built Arul after interviewing 3,000\+ patients and caregivers, is assembling a talent-dense founding team, and we are looking for a true architectural heavyweight to serve as its technical backbone.
The Role
This is a high-ownership, principal-level position for a builder who wants to shape the technical architecture, the product experience, and the company culture. You will join a small, talent-dense team and be responsible for keeping it that way: owning end-to-end technical decisions, building alongside our founding engineer and offshore team, and establishing an uncompromising engineering bar that makes top-tier talent excited to join.
You will join as our Head of Engineering, with a massive trajectory to define our engineering culture and scale into a broader technical leadership role as we expand.
What You Will Do
- Architect the Foundation:
Own cross-system technical decisions across the entire stack: the application layer, high-scale data pipelines, cloud infrastructure, and the core LLM orchestration layer.
- Build Hands-On:
Ship production code, debug complex live issues, and ensure system stability while laying a zero-technical-debt groundwork for scale.
- Drive the AI Product:
Lead development of our agentic, LLM-centric product, with advanced agent orchestration built on frameworks like LangGraph and Google's Agent Development Kit (ADK) and rigorous, deterministic prompt engineering.
- Own the Product Experience:
Bring real product design sensibility. Translate deep patient insight into intuitive, polished UI/UX and hold a high bar for craft across the product.
- Secure the Platform:
Design and implement healthcare-grade security and compliance protocols, ensuring readiness for HIPAA, SOC 2, and enterprise-grade software standards from day one.
- Lead Technical Partnerships:
Act as the definitive technical authority with senior technical stakeholders at major health systems (e.g., Northwestern Medicine), navigating complex API integrations and security reviews.
- Build a Talent-Dense Team:
Mentor a small, fast-moving engineering team, unblock offshore resources, and own our technical hiring and interview processes, recruiting people who raise the bar.
What We Are Looking For
- Experience:
3-12 years of software engineering experience, with a proven track record of operating at a Staff or Principal level in a top-tier engineering organization.
- Full-Stack Expertise:
Proven end-to-end product development spanning frontend, backend, infrastructure, and security.
- Agentic AI Fluency:
Hands-on experience with agent frameworks such as LangGraph or Google's Agent Development Kit (ADK), LLM orchestration, and complex multi-agent systems.
- Product \& Design Sense:
Strong product design and UI/UX instincts. You sweat the details of the user experience and can take a feature from concept to something beautiful.
- Regulated Environments:
Experience building in healthcare or similarly complex, highly regulated environments (HIPAA / SOC 2 strongly preferred).
- Pedigree:
A background at an elite tech company, a strong healthcare product company, or a premier academic institution.
- Builder Mentality:
The strategic vision to set long-term technical direction, coupled with the grit to dive deep into the codebase and fix production issues directly.
- Cultural Add:

- Location:
Must be based in Boston, New York, or elsewhere on the East Coast.
