Occupational Therapist - Full Time

Role Overview

Provides specialized occupational therapy to improve patients' physical, social, and mental functions within a home health or hospice setting. Acts as co-leader in interdisciplinary teams to develop and implement individualized treatment plans, ensuring quality care and proper discharge procedures.

Responsibilities

• Conduct assessments and develop personalized treatment strategies.
• Deliver therapy interventions using established protocols.
• Complete accurate clinical documentation and communicate plans effectively.
• Supervise assigned assistants and perform additional duties as needed.

Qualifications

• Master's or Doctorate in Occupational Therapy with applicable licensure in practice states.
• CPR/BLS certification.
• Valid driver’s license; minimum age 21\.
