Job Description
Role Summary:
In this role, you will evaluate and provide therapeutic interventions to patients, helping them regain independence and improve their quality of life. You will play a crucial role in developing and overseeing functional maintenance programs and collaborating with a multidisciplinary team to ensure the best possible patient outcomes.
Duties \& Responsibilities:
- Conduct comprehensive assessments of patients and develop individualized care plans.
- Implement and oversee therapeutic exercise programs to enhance patients' functional abilities.
- Regularly reassess patients to monitor progress and adjust treatment plans accordingly.
- Coordinate with team members to ensure seamless patient care and integration of therapy into overall health plans.
- Educate patients, families, and caregivers on rehabilitation strategies and home-based interventions.
- Provide training to staff and caregivers on therapy techniques that can be incorporated into daily care.
- Maintain detailed and accurate documentation of therapy sessions, progress notes, and assessments.
- Ensure all therapy treatments align with industry best practices and regulatory standards.
- Participate in team meetings and contribute to comprehensive patient care planning.
- Stay current with advancements in occupational therapy and participate in ongoing professional development.
- Assist in the selection and installation of adaptive equipment to enhance patient safety and independence.
Required Qualifications:
- Graduate of an Occupational Therapy program accredited by the American Occupational Therapy Association.
- Valid and current Occupational Therapy License in good standing.
- Minimum of 2 years of clinical experience working with adults with physical disabilities.
- Reliable transportation, valid driver’s license, and state-required auto insurance.
- Must be medically cleared for communicable diseases and have up-to-date immunizations before engaging in direct patient contact.
- Ability to pass a comprehensive background check, including criminal history, license verification, and drug screening.
Desired Qualifications:
- Advanced practice certification in dysphagia (preferred but not required).
- At least one year of experience working with a frail or elderly population (training will be provided if not met).
- Experience with adaptive therapy equipment and assistive technology for individuals with physical disabilities.
- Strong teamwork and communication skills to collaborate effectively with a multidisciplinary team.
- Proficiency with electronic health records and basic computer skills.
Work Environment \& Physical Demands:
- Hands on patient care in a rehabilitation gym and adult health center setting.
- Frequent lifting, pushing, pulling, kneeling, and standing for extended periods.
- Collaboration with healthcare professionals in a moderately noisy clinical environment.
- Occasional home visits to assist with adaptive equipment installation and patient support.
