Midland Care Connection
Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas.
PACE Program
The PACE (Program of All-Inclusive Care for the Elderly) program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.
Intake \& Enrollment Manager
The Intake \& Enrollment Manager leads the PACE intake and enrollment process from the initial inquiry or referral through enrollment. They supervise and support the Intake Coordinators, ensure prospective participants and their families receive timely and accurate information, and work with the PACE team to remove barriers that could delay enrollment. The manager also tracks enrollment activity and performance metrics, monitors the referral pipeline, identifies opportunities to improve the process, and develops strategies to help meet enrollment and growth goals. This role works closely with Marketing, Center Administration, and the Interdisciplinary Team to ensure a smooth participant experience while maintaining compliance with Medicare, Medicaid, PACE, and confidentiality requirements.
Required
- Bachelor’s degree in healthcare administration, business, social services, nursing, or a related field, or equivalent combination of education and experience
- Previous experience in healthcare, senior services, PACE, case management, intake, enrollment, or a related setting
- Previous supervisory or leadership experience, including coaching, training, and performance management
- Strong organizational and time-management skills with the ability to manage multiple prospective participants and competing priorities
- Excellent verbal and written communication skills
- Ability to build positive relationships with prospective participants, families, referral sources, and interdisciplinary teams
- Strong problem-solving and critical-thinking skills with the ability to identify and remove barriers to enrollment
- Proficiency with electronic health records, databases, Microsoft Office, and other technology used to track and manage enrollment activity
- Ability to maintain confidentiality and handle HIPAA-protected information appropriately
- Ability to understand and follow Medicare, Medicaid, PACE, and other applicable eligibility and enrollment requirements
Preferred
- Experience working directly with a PACE program
- Experience managing an intake, admissions, enrollment, referral, or business development team
- Knowledge of Medicare and Medicaid eligibility requirements and PACE enrollment processes
- Experience using enrollment metrics, dashboards, or other data to monitor performance and identify trends
- Experience developing or improving intake and enrollment processes
- Experience collaborating with Marketing, clinical teams, and community referral partners to support growth
- Experience with community outreach, presentations, or relationship development
- Knowledge of the healthcare needs and services of older adults and individuals with complex care needs
- Previous experience working with an interdisciplinary team
