As Director of Primary Care, you’ll own a geographic region end-to-end — its performance, its people, and its potential. This is not a role for someone who wants to maintain; it’s for someone who wants to transform. You’ll drive the transition to value-based care, lead a diverse team of clinic managers and providers, and create care experiences that patients remember for all the right reasons.

You’ll have real authority and real accountability: full P\&L ownership, a seat at the strategic table, and the autonomy to design regional initiatives that move the needle.

Compensation

This position offers a salary of

$140k\+

annually, commensurate with experience, plus a comprehensive benefits package. Final compensation reflects the candidate’s background, qualifications, and alignment with the scope of the role.

Key Responsibilities

  • Design and execute region-specific objectives that advance corporate strategy
  • Attract and recruit medical providers across employment, partnership, or management models
  • Use predictive analytics, AI dashboards, and operational data to drive informed decisions
  • Supervise and develop clinic managers, nurse care coordinators, and physicians/APPs
  • Manage a regional budget with full P\&L accountability and a focus on cost containment
  • Monitor local, state, and national healthcare shifts — then act on them proactively
  • Ensure compliance with Certified Rural Health Clinic (RHC) requirements
  • Ensure compliance with Patient Centered Medical Home (PCPCH) model standards
  • Drive successful execution of quality-based payment models
  • Represent Praxis Health with local and state agencies
  • Champion continuous improvement and innovative process adoption across your region
  • Develop and implement strategic initiatives in alignment with executive leadership

Qualifications

Minimum Education

Bachelor’s degree in Healthcare Administration, Business Administration, Health Administration, or equivalent — or a licensed healthcare professional with relevant management experience.

Minimum Experience

5\+ years managing employees in a clinic setting or equivalent; proven track record of leading teams through change and organizational development.
