Responsibilities

• Perform initial PT evaluations and develop individualized treatment plans for residents in a skilled nursing setting.
• Deliver therapeutic interventions to improve mobility, gait, balance, strength, and functional independence.
• Reassess progress regularly and document outcomes in the patient record, updating plans as needed.
• Collaborate with an interdisciplinary team (PT assistants, nursing staff, physicians) to coordinate care and support discharge planning.
• Educate residents and families on home exercise programs, safety strategies, and injury prevention.
• Ensure patient safety, proper use of equipment, and adherence to infection control and privacy standards.

Requirements

• Ability to work in a skilled nursing facility environment
• Willingness to complete a travel assignment of approximately 13 weeks
• Strong communication skills and ability to document clearly
• Current, active Physical Therapist license (in good standing) and eligibility to practice in the United States
• Commitment to professional standards and facility policies
