Job Overview
An exciting opportunity has arisen for a Senior Clinical Fellow to join our Community Care of the Elderly team based in Ebbw Vale.
As a team member, you will provide senior clinical input to our community wards, working closely with the wider multidisciplinary team to deliver safe, person-centred care for older adults. The role offers hands-on clinical experience across the community inpatient setting in Ysbyty Aneurin Bevan hospital supporting management of the older adult and discharge planning and supporting with collaborative decision-making with nursing, therapy, pharmacy and social care colleagues.
This post is ideal for a doctor with experience in elderly care or general medicine who is keen to develop their skills in community geriatrics. This post provides the opportunity to join a supportive team and gain exposure to innovative models of integrated care.
Main duties of the job
The successful candidate will be a valuable and senior member of the Community COTE Directorate and will be responsible to the senior physician to whom they are working. The expectation of the role is to provide senior support as part of a multi-disciplinary team. The post holder will be expected to participate in outpatient clinics related to the specialty and take part in multidisciplinary meetings within the department. The post holder will carry out independent ward rounds with senior support available, where needed.
The Senior Clinical Fellow post will involve providing active medical input to patients admitted to the ward for rehabilitation, stroke rehabilitation or palliative care needs. The Senior Clinical Fellow will be responsible for the 16 medical beds on Tyleri Ward under the direction of the senior clinician. The post holder will work closely with the community hospital teams across Ebbw and Sirhowy Wards, the Blaenau Gwent Community Resource Team and the wider nursing staff.
