About The Role
We are seeking a dedicated and compassionate Healthcare Coordinator to join our team at the Center for Elders' Independence. In this role, you will be responsible for managing and coordinating various aspects of elder care services, ensuring seamless communication between clients, families, and healthcare providers. The ideal candidate will possess excellent organizational skills, a deep understanding of elder care needs, and a commitment to delivering exceptional service. Qualifications
- Bachelor’s degree in Nursing, Social Work, Healthcare Administration, or a related field.
- Minimum of 3 years of experience in elder care, healthcare coordination, or case management.
- Strong knowledge of healthcare regulations, elder care best practices, and community resources.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Proficiency in electronic health records (EHR) systems and Microsoft Office Suite.
- Empathy, patience, and a genuine desire to improve the lives of seniors.
Responsibilities
- Coordinate and oversee the delivery of elder care services, ensuring that care plans are tailored to individual needs and preferences.
- Act as a primary point of contact for clients, families, and healthcare providers, facilitating effective communication and addressing any concerns promptly.
- Assess client needs through home visits, interviews, and medical records review to develop personalized care strategies.
- Monitor and evaluate the effectiveness of care plans, making adjustments as necessary to optimize outcomes.
- Ensure compliance with healthcare regulations, licensing requirements, and organizational policies.
- Assist in the onboarding and training of new staff and caregivers to maintain high standards of service delivery.
- Maintain accurate and up-to-date documentation of client interactions, care plans, and service activities.
- Participate in community outreach and education initiatives to promote elder health and wellness programs.
- Collaborate with multidisciplinary teams to ensure holistic and coordinated care for clients.
