Quality Life Healthcare is seeking a compassionate, organized, and clinically experienced Registered Nurse (RN) or Nurse Practitioner (NP) to join our team as a Hybrid Clinical Assessor for our private duty home care division.

This position combines both remote and in-office responsibilities and focuses on conducting clinical assessments, care planning, participant evaluations, and oversight for private pay home care clients. The ideal candidate will have strong assessment skills, excellent communication abilities, and experience working with seniors, chronic conditions, memory care, and home care services.

The Clinical Assessor will play a key role in helping families determine appropriate levels of care while ensuring quality, safety, and continuity of services for participants receiving care in the home setting.

Responsibilities

  • Conduct initial and ongoing clinical assessments for private duty home care clients
  • Perform remote assessments via phone or telehealth platforms when appropriate
  • Conduct in-home assessments as needed
  • Develop individualized care plans based on participant needs and safety concerns
  • Evaluate functional status, cognitive concerns, fall risks, medication management needs, and overall home safety
  • Collaborate with caregivers, families, office staff, and community providers regarding participant care needs
  • Review and update service recommendations as participant conditions change
  • Provide clinical guidance and recommendations regarding appropriate home care services
  • Maintain accurate and timely documentation of assessments, care plans, and participant interactions
  • Participate in case conferences and quality assurance activities
  • Assist with caregiver education and support as needed
  • Support participant admissions and service coordination processes
  • Ensure compliance with agency policies, HIPAA requirements, and applicable State regulations

Qualifications

  • Active Pennsylvania RN or NP license in good standing
  • Minimum of 2 years of nursing experience preferred
  • Experience in home care, geriatrics, hospice, case management, or private duty services preferred
  • Strong assessment and clinical documentation skills
  • Experience working with seniors and individuals with chronic conditions or memory impairment preferred
  • Comfortable conducting telehealth and remote assessments
  • Excellent communication and interpersonal skills
  • Strong organizational and time management abilities
  • Ability to work independently and collaboratively
  • Proficiency with electronic documentation systems and basic computer applications

Physical Requirements

This hybrid position offers flexible scheduling opportunities, including remote work from home for approved assessment and documentation activities.

Compensation is based on credentials, experience, and scope of responsibilities.
