Duties
The Community Program Manager leads the successful implementation of a statewide, community-driven Integrated Care High Risk (ICHR) Program focused on advancing upstream, equitable solutions to complex community challenges. This role is responsible for designing, coordinating, and facilitating Wilder’s community initiative, working closely with internal teams, community partners, and key stakeholders to support program development, partner selection, and ongoing implementation. In collaboration with the Director and cross-sector partners, the Program Manager helps guide strategy and execution while ensuring strong alignment across efforts. The ideal candidate is highly self-directed, relationship-oriented, and brings established community connections from day one. Additionally, this role oversees collaborative facilitation and partnership efforts by providing consultation, training, and capacity-building support to community and government partners working to improve high-risk pregnancy outcomes and overall family well-being.
Job Qualifications
Bachelor’s degree required. At least 2-3 years of experience working with community. Must have strong collaboration, facilitation and training experience. Previous experience with community engagement and innovative design is preferred. Must demonstrate cultural competence and experience working with people from diverse backgrounds.
