Nurse Practitioner (NP) / Physician Assistant (PA) – Pathfinder Recovery

Reports To: Chief Medical Officer (CMO)

Position Overview:

The Nurse Practitioner (NP) or Physician Assistant (PA) at Pathfinder Recovery will play a key role in telehealth-based addiction and mental health treatment services, providing medical assessments, medication-assisted treatment (MAT) management, and ongoing patient care. This provider will work within a multidisciplinary team to support Virtual Ambulatory Detox, MAT, Partial Hospitalization Program (PHP), Intensive Outpatient Program (IOP), and Outpatient (OP) services.

As part of a technology-driven addiction recovery model, the NP/PA will leverage telehealth tools, biometric monitoring, and AI-powered patient tracking to provide personalized, evidence-based care. This role is ideal for a clinician passionate about expanding access to addiction treatment through innovative, patient-centered telehealth solutions.

Key Responsibilities:

Patient Care \& Medical Assessments

  • Conduct comprehensive telehealth-based medical assessments for patients entering Virtual Ambulatory Detox, MAT, or outpatient treatment programs.
  • Diagnose and manage substance use disorders (SUDs), co-occurring mental health conditions, and withdrawal symptoms using ASAM best practices.
  • Develop individualized treatment plans, ensuring patients receive safe and effective medication-assisted treatment (MAT) when appropriate.
  • Monitor patient progress, medication adherence, and symptom management through telehealth consultations.
  • Provide crisis intervention support and coordinate with emergency medical services (EMS) or in-person care when necessary.

Medication-Assisted Treatment (MAT) Management:

  • Prescribe and monitor FDA-approved medications (buprenorphine, methadone, naltrexone) for opioid and alcohol use disorders.
  • Adjust medication dosing based on patient response, lab results, and adherence data from biometric monitoring tools.
  • Educate patients on medication safety, side effects, and harm reduction strategies.
  • Ensure compliance with DEA prescribing regulations and SAMHSA guidelines for MAT programs.

Telehealth \& Digital Health Integration:

  • Utilize HIPAA-compliant telehealth platforms for secure virtual consultations, medication management, and therapy coordination.
  • Leverage biometric monitoring (Pretaa), AI-driven relapse prevention tools, and digital treatment tracking systems to optimize patient care.
  • Coordinate with therapists, peer recovery coaches, and case managers to ensure seamless, integrated patient support.
  • Ensure accurate EHR documentation of patient encounters, medication prescriptions, and clinical progress.

Compliance \& Quality Assurance:

  • Adhere to ASAM, SAMHSA, HIPAA, DEA, and state-specific regulations for telehealth addiction treatment.
  • Participate in case reviews, peer consultations, and clinical quality improvement initiatives.
  • Maintain licensure in applicable states (Connecticut, Massachusetts, Vermont) and stay updated on addiction medicine best practices.
  • Work with the Clinical Director and CMO to enhance treatment protocols, risk assessment tools, and patient safety measures.

Qualifications \& Experience:

Required Experience:

  • Active NP or PA license in Connecticut, Massachusetts, New Hampshire and/or Vermont (or eligibility for licensure).
  • 3\+ years of clinical experience, with at least 2 years in addiction medicine, behavioral health, emergency medicine, or primary care.
  • DEA certification (or ability to obtain) for prescribing buprenorphine/naltrexone for MAT programs.
  • Strong understanding of ASAM criteria, MAT protocols, and best practices for telehealth addiction treatment.
  • Experience using electronic health records (EHR), telehealth platforms, and virtual patient engagement tools.

Preferred Experience:

  • X-waiver (DATA 2000 certification) for prescribing buprenorphine (preferred but not required).
  • Experience in ambulatory detox, emergency medicine, or crisis intervention.
  • Knowledge of AI-powered patient tracking, biometric monitoring, and digital health innovations in addiction care.
