As a Home Health Occupational Therapist, you'll help patients overcome physical and functional challenges while improving their ability to perform daily activities safely and independently.

What You'll Do

• Complete occupational therapy evaluations, reassessments, and discharge visits
• Develop individualized treatment plans based on patient goals and needs
• Provide skilled occupational therapy services in patients' homes
• Assess home safety and recommend adaptive equipment and environmental modifications
• Educate patients and caregivers on techniques that promote independence and safety
• Collaborate with physicians and interdisciplinary team members
• Monitor patient progress and adjust treatment plans as needed
• Maintain timely and accurate clinical documentation

Qualifications

• Active Pennsylvania Occupational Therapist License Required
• Valid Driver's License and Reliable Transportation
• Strong Communication and Documentation Skills
• Home Health Experience Preferred but Not Required
• Commitment to Delivering Exceptional Patient-Centered Care
