Position Overview
An Occupational Therapist in home health provides one-on-one, flexible scheduling care to help patients live independently. Key responsibilities include performing assessments of mobility, strength, and function; developing and implementing personalized care plans; documenting progress; and collaborating with healthcare teams and family members. The role involves discharge planning, education, and supervising other personnel like OTA and HHA. The therapist must ensure documentation compliance, participate in service coordination, and complete assessments such as OASIS where permitted.
Qualifications \& Skills
- Degree in Occupational Therapy from an accredited program
- Current, unrestricted state license (WI and MN)
- Proficiency in assessments, documentation, and policy compliance
- Valid CPR and driver's license
