Job Description

Position Summary:

Under the supervision of the Chief Operations Officer, the Behavioral Health Program Manager serves as a subject matter expert for mental healthcare and behavioral health programs. The Program Manager will carry an individual caseload as well as provide staff supervision, strategic programmatic design and organizational leadership to ensure high-quality behavioral health outcomes for Affinity Health Center patients. This role is a valuable member of AHC leadership team and will ensure strong interdepartmental relationships between behavioral health, case management, AHC clinic, accounting, and patient access, teams.

Duties \& Responsibilities:

Direct Clinical Services/Integrated Care:

  • Provide psychotherapy services to individuals, families, and groups across the lifespan.
  • Conduct ongoing assessments of patients? mental health, substance use, and psychosocial needs.
  • Develop, implement, and regularly update individualized treatment plans that reflect patient goals, presenting symptoms, and progress.
  • Evaluate the effectiveness of treatment plans and adjust interventions or level of care as clinically indicated, including short-term care, ongoing therapy, or discharge from the behavioral health program.

Clinic \& Staff Supervision:

  • Provide clinical supervision, coaching, and support to the behavioral health team.
  • Monitor staff productivity, documentation quality, and performance metrics.
  • Participate in performance management activities, including evaluations and corrective action when needed.

Administrative/Leadership:

  • Works with senior leadership to ensure standard operating procedures and policies related to Behavioral Health Program and care delivery are up to date and compliant.
  • Provide appropriate training to staff on patient care, standard operating procedures, documentation, billing, organizational and clinic policies, etc.
  • Work with data analysts to monitor key performance indicators, analyze trends, and establish goals to improve patient outcomes, and monitors quality measures and care gaps and implement strategies to improve outcomes.
  • Participate in continuous quality improvement projects to reach organizational quality metrics and/or infrastructure improvement projects.
  • Support budget projections and monitoring.
  • Collaborate with the revenue cycle team to ensure appropriate coding, documentation, and financial accountability.
  • Works closely with Associate Medical Director and psychiatric nurse practitioners to ensure continuity of care between clinical programs.
  • Represent AHC at community and professional networks meetings.
  • Build and maintain relationships with local school districts, referral, peer and partner organizations.
  • Strategically expand service offerings, improve quality and efficiency through systematic improvements and monitoring.
  • Support Senior Leadership Team with application for, and compliance with, AAAHC, FTCA, PCMH, UDS and other certification or regulatory requirements.
  • Serve as a member of the AHC Leadership Team.
  • Other related duties as assigned.

Pay Range:

  • $70,000 ? $88,000 annually based on qualification and experience.

Preferred Qualifications

  • Ability to speak in English and Spanish
  • Thorough knowledge of HIV
  • LPC-S or LISW-CP or LISW- AP
  • LCAS, LAC, CADC or similar
