What To Expect (Job Responsibilities)

• Build and maintain trusting relationships with patients, families, and healthcare providers
• Engage with high-risk patient populations to address health questions and care needs
• Assist patients in navigating their care, including referrals and appointment scheduling
• Document all interactions and assessments accurately in electronic health records
• Collaborate with interdisciplinary teams to meet Key Performance Indicators (KPIs)

What Is Required (Qualifications)

• Certified or Registered Medical Assistant OR Certified EMT with 2 or more years of experience
• 2-3 years of experience in patient navigation, medical assisting, or care coordination
• Experience working directly with patients in clinical or non-clinical settings
• Demonstrated proficiency with EHR documentation and remote work effectiveness
• Bilingual in English and Spanish strongly desired

How To Stand Out (Preferred Qualifications)

• Patient Navigation certification or willingness to obtain within 1 year
• Bachelor's Degree in a health-related field
• Experience with motivational interviewing and critical listening skills
• Familiarity with various EHR systems such as Epic, Athena, and Cerner
• Strong understanding of social determinants of health
