Family Care APP (NP or PA)

We are seeking an Advanced Practice Provider to deliver comprehensive outpatient family medicine care at our Prineville Clinic, under supervision of a physician.

• Provide evaluation, diagnosis, treatment, and documentation for patients of all ages, coordinating with multidisciplinary teams within a Medical Home model.
• Create integrated treatment plans, monitor ongoing care, and refer patients to specialists as needed.
• Participate in call rotations, support quality improvement, and ensure timely documentation and patient education.
• Maintain licensure, certifications, and compliance with all policies and regulations.

Qualifications include:

a Master’s degree in Physician Assistant Studies or Nursing, licensure, certification (NCCPA or AANC/ANNP), experience in outpatient family medicine, and proficiency with EHR systems. Preferred candidates have 2\+ years of experience, familiarity with the Medical Home model, and Epic software experience.
