Job Description
**We are seeking a dedicated
Occupational Therapist (OT)
to assess, plan, and deliver therapy services in home health settings.**
Key Responsibilities Include
- Conduct initial and ongoing assessments of patients’ mobility, strength, and function.
- Develop and implement personalized plans of care with measurable goals and outcomes.
- Provide therapy interventions following physician orders and document progress accurately.
- Coordinate with healthcare team members, patients, and caregivers to optimize care.
- Manage discharge planning and educate patients on home exercise programs.
- Supervise other personnel such as OT Assistants and Home Health Aides where applicable.
- Complete assessments within regulatory guidelines and maintain compliance with documentation policies.
Qualifications And Skills Required
- Degree in Occupational Therapy from an accredited program.
- Current, unrestricted state license as an OT.
- Valid CPR certification, driver’s license, and automobile insurance.
