**We are seeking a dedicated

Occupational Therapist (OT)

to assess, plan, and deliver therapy services in home health settings.**

Key Responsibilities Include

• Conduct initial and ongoing assessments of patients’ mobility, strength, and function.
• Develop and implement personalized plans of care with measurable goals and outcomes.
• Provide therapy interventions following physician orders and document progress accurately.
• Coordinate with healthcare team members, patients, and caregivers to optimize care.
• Manage discharge planning and educate patients on home exercise programs.
• Supervise other personnel such as OT Assistants and Home Health Aides where applicable.
• Complete assessments within regulatory guidelines and maintain compliance with documentation policies.

Qualifications And Skills Required

• Degree in Occupational Therapy from an accredited program.
• Current, unrestricted state license as an OT.
• Valid CPR certification, driver’s license, and automobile insurance.
