Job Summary
We are seeking a dedicated and energetic Infusion Registered Nurse Care Manager to join our dynamic healthcare team. In this vital role, you will coordinate and oversee infusion therapy services, ensuring seamless patient care from admission through discharge. Your expertise will help optimize treatment plans, enhance patient outcomes, and promote efficient utilization of healthcare resources. As a key member of our multidisciplinary team, you will collaborate closely with physicians, pharmacists, and other healthcare professionals to deliver exceptional care in various settings including hospital, outpatient clinics, and home health environments. Your proactive approach and clinical knowledge will drive patient-centered solutions that improve quality of life and foster positive health experiences.
Responsibilities
- Assess patient needs through comprehensive clinical evaluations, vital signs monitoring, and review of medical records to develop personalized infusion care plans.
- Manage patients receiving complex infusion therapies, including biologics, antibiotics, chemotherapy, and hydration treatments across inpatient and outpatient settings.
- Collaborate with healthcare professionals to coordinate discharge planning, post-acute care, and home health services to ensure continuity of care.
- Utilize electronic health records (EHR) systems to document patient interactions, treatment progress, and medical records accurately while maintaining HIPAA compliance.
- Conduct case management activities such as patient assessment, diagnostic evaluation, utilization management, and clinical case documentation to optimize treatment efficacy.
- Educate patients and their families on infusion procedures, medication administration, potential side effects, and safety protocols to promote adherence and understanding.
- Participate in quality improvement initiatives by adhering to hospital policies, health regulation standards, and infection control practices related to infusion therapy.
Qualifications
- Serve as the primary contact for patients, responding to inquiries with clarity, empathy, and efficiency.
- Educate patients and providers on home infusion processes, including medication handling, storage, and administration protocols.
- Schedule and confirm treatment appointments, coordinating with nursing agencies to ensure timely, accurate care delivery.
- Verify medication receipt and comprehension with patients prior to treatment.
- Ensure clinical documentation is shared with providers post-infusion and that billing information is properly routed to the finance team.
- Collaborate cross-functionally with the Customer Success team to maintain strong client relationships and support service excellence.
Pay: From $80,000\.00 per year
