Job Description
Role:
Nurse Practitioner, Advanced Practice Provider
Responsibilities
- Provide effective, equitable, value-based primary care to adult Medicare patients
- Conduct office visits for routine and acute issues
- Administer wellness visits, health risk assessments, and care coordination
- Build meaningful patient relationships and educate on health conditions and treatments
- Participate in population health initiatives and outreach activities
- Collaborate with a multidisciplinary care team and support providers
- Perform home visits as needed
Qualifications \& Skills
- Master of Science in Nursing, Doctor of Nursing Practice, or NP certification
- Active, non-probationary state NP license and DEA license
- National certification in family, adult-gerontology, adult, or gerontological nurse practitioner
- Experience in primary care, understanding of HCC, ICD-10 coding, and HRAs (preferred)
- Passion for teamwork and population health
Additional Details
- Full-time role with approximately 40 hours per week
- Salary range: $104,442 - $224,952, dependent on experience and location
