Nurse Practitioner, Advanced Practice Provider

Role:

Nurse Practitioner - Advanced Practice Provider (Bilingual Spanish Preferred)

Responsibilities

• Provide effective, equitable, value-based primary care to adult Medicare patients
• Conduct office visits for routine and acute issues
• Administer wellness visits and health assessments with thorough documentation
• Coordinate care with other providers, specialists, and testing facilities
• Lead population health initiatives and ensure preventive screenings
• Assist with phone triage, patient education, and outreach efforts
• Participate in promotional activities and conduct home visits as needed
• Collaborate within a multidisciplinary outpatient team and report to the Center Medical Director

Qualifications \& Skills

• Master of Science in Nursing, Doctor of Nursing Practice, or NP certification
• Active state Nurse Practitioner license and DEA license
• Certifications in Family, Adult-Gerontology, or Gerontological Nurse Practice
• Experience in primary care, geriatrics, or internal medicine (preferred)
• Knowledge of HCC, ICD-10 coding, and health risk assessments (preferred)
• Strong teamwork, communication, and patient relationship skills
• Bilingual proficiency in applicable areas is a plus
