Job Description
Join a team dedicated to providing comprehensive in-home health assessments that improve patient outcomes and close care gaps.
Responsibilities include:
- Conducting adult and geriatric health assessments, reviewing medical history, medications, and social factors
- Administering and ordering point-of-care and lab testing
- Collaborating with physicians, case managers, social workers, and caregivers
- Providing patient education and follow-up, often at members' homes or facilities
Qualifications:
- Master’s Degree or equivalent with NP licensure and prescriptive authority
- Board certification in relevant specialties (e.g., Adult Gerontology, Family, Emergency)
- At least 1 year of NP experience, current BLS/ACLS or CPR certification
- Ability to travel extensively, valid driver’s license, and automobile insurance
- Strong communication skills and computer proficiency
